Case Management: Includes a functional needs assessment, development of a care plan, arrangement for services and on-going monitoring of recipient to assure stabilization.
Homemaker: Clients are provided assistance with light cleaning, meal preparation and shopping.
Personal Care: Clients receive assistance with activities of daily living such as bathing, dressing, ambulation housekeeping, personal hygiene needs, and/or supervision of self-administered medications.
Respite: Client caregivers are provided time away from demands of providing care.
Live-In: Clients requiring supervision are provided live-in companion/personal care worker around-the-clock.
Adult Day Health Care: Clients attend Day Care 1-5 days per week in order to receive therapy (disabled adults age 18-59 only) with therapeutic, social, and health activities.
Medical Supplies & Equipment: Clients can receive supplies and equipment on a cost by cost basis.
